Family Support 360

The Family Support 360° Program provides family-focused services and support to improve the quality of life for families caring for people with developmental disabilities.

Navigators, who are local to the communities they serve, work with families to identify their unique needs, develop an Individualized Family Plan, and ensure families receive the services and support they require.

Set Goals & Develop a Plan

· Develop a comprehensive Family Individualized Plan to prioritize your family’s needs, set objectives and work toward achieving them

Get Connected

· Take advantage of opportunities to meet other families, support each other and share information

Gain Information

· Gain access and utilize community, state & federal resources, public services, disability information

· Get assistance to navigate the service system

Expand your knowledge & skills

· Attend trainings & workshops on topics important to you and other families
