Adolescent Health Transition Project
The Adolescent Health Transition Project is designed to help smooth the transition from pediatric to adult health care for adolescents with special health care needs. This site is a resource for information, materials, and links to other people with an interest in health transition issues.

The Center for Change in Transition Services
The Center for Change in Transition Services, Seattle University, is a state-needs grant funded by the Office of the Superintendent of Public Instruction. 

Job Accommodation Network (JAN)
The Job Accommodation Network (JAN) is a free consulting service that provides information about job accommodations, the Americans with Disabilities Act (ADA), and the employability of people with disabilities.

National Center on Secondary Education and Transition
The National Center on Secondary Education and Transition (NCSET) was established to create opportunities for youth with disabilities to achieve successful futures. Headquartered at the Institute on Community Integration, University of Minnesota.

Transition Coalition
The mission of the Transition Coalition is to maximize professional development in secondary school reform and
transition at the national, state, and local levels. The Coalition supports best practices and creates professional
development forums using face-to-face and online training and technical assistance.

Transition Link
Transition link is an on-line community for sharing ideas, strategies, resources, and information concerning the
transition to life after high school for adolescents with disabilities.

Transition Research Institute at Illinois  (TRI)
The Transition Research Institute at the University of Illinois (TRI), established in 1985, identifies effective
practices, conducts intervention and evaluation research, and provides technical assistance activities that
promote the successful transition of youth with disabilities from school to adult life.

National Center on Secondary education and Transition

The National Center on Secondary Education and Transition (NCSET) coordinates national resources, offers technical assistance, and disseminates information related to secondary education and transition for youth with disabilities in order to create opportunities for youth to achieve successful futures.
